
Medical clearance tools for participation in the FAME program
Before starting FAME, it is important to ensure that participants are medically cleared to engage in physical activity safely. Exercise professionals and recreation facilities commonly use medical screening tools to identify potential health risks and determine whether additional medical clearance is needed prior to participation.

Physical Activity Readiness Questionnaire (PAR-Q & You) Click to download
- The PAR-Q is a widely used self-screening questionnaire designed to identify individuals for whom physical activity might be inappropriate or who may require medical clearance before exercising. It asks about existing medical conditions, symptoms, and health history related to exercise participation.
This tool is commonly used in fitness and recreation settings in Canada. PAR-Q & YOU is the original 7-question version designed for individuals aged 15-69.

- An updated, 4-page PAR-Q version designed for all ages. It includes follow-up questions for those with chronic conditions to help screen people in to exercise rather than automatically referring them to a doctor.

- This is a health-screening tool tailored for stroke survivors that identifies medical risks, such as heart conditions, osteoporosis, and respiratory issues, before they begin the program. It serves as a safety check to ensure participants can safely increase physical activity or determines if they need professional medical clearance first.
Get Active Questionnaire (GAQ) Click to download
- The GAQ is a screening tool developed by the Canadian Society for Exercise Physiology. It is designed to be a self-administered and support safe participation in physical activity by identifying potential medical considerations and guiding next steps if further clearance is required.
811 Physical Activity Services Line (BC Residents Only)
- For individuals who are unable to obtain medical clearance from a physician or other health professional, HealthLink BC’s 811 Physical Activity Services Line offers an alternative screening option.
- By calling 811, BC residents can speak with a qualified exercise professional who can help assess readiness for physical activity and provide guidance related to medical preparedness.
- This service is available to BC residents only and requires a valid BC personal health number.
The FAME program does not require a specific screening tool. Organizations and exercise professionals may choose the screening approach that best aligns with their policies, professional standards, and liability requirements. The tools listed above are provided for informational purposes to support safe and informed participation in physical activity.
